Thank you for being a part of COA Elite. We know at times things can happen and you must cancel your membership. To do so this form must be completed and submitted to our office no less than 30 days PRIOR to the last attended class to avoid being billed for additional tuition payments. If the form is not received 30 days prior, you will be responsible for the final month of tuition for the dropped class(es) regardless of attendance.
